A group bringing RTM to several clinics or a hospital network faces a different task than a single practice. More roles are involved, workflows must align, and more staff need training before enrolling the first patient. Actuvi handles all preparation and planning and standardizes it across sites so no clinic runs its own separate program.
What does a multi-site group need to decide first?
The first conversation is about the program's goals and what success looks like. Actuvi measures the rollout against how the group will judge success, so everyone knows how the program is evaluated.
Goals can differ by site. One clinic may focus on clinical improvement, another on patient engagement, and another on cleaner data for reporting. These goals sit alongside the program's therapeutic areas, such as pain management, mental health support, or respiratory care. A group with a surgery center in one location and a specialist practice in another usually has more than one set of goals, which is expected.
Which care pathways exist at each location?
Next, Actuvi maps the care pathways each site handles. Some locations treat one kind of patient; others handle several. Actuvi takes a comprehensive approach, building the program from assessments to monitoring tracks so the platform supports every pathway the group runs.
Actuvi offers assessment templates and full customization so a site can start from a proven template or build its own. The goal is to build an RTM program that fits how each clinic treats patients while staying consistent across the group.
How is the RTM program structured across sites?
The typical structure has one administrator for the whole group, with each clinical site having its own site manager, and onboarding and monitoring personnel reporting to them.
The biggest decision in that structure is how to monitor. A group can have staff at each site monitor their own patients or set up one central person or team to monitor all sites.
Which patients should be enrolled?
A group can enroll all patients or start with a subset. They might choose a subset by age, condition, or insurance provider. Knowing who the program is for makes it easier to design onboarding workflows, estimate how many patients each site will enroll, and how long each patient is likely to stay.
Should patients be imported in bulk or added as they arrive?
Actuvi can bulk-import an existing patient list, or clinics can create patients one at a time as they come in. Some groups also bring in new patients through EHR integration. The right choice depends on how many patients the group wants to start with and how each clinic schedules its day. It can differ from site to site.
After enrollment, Actuvi sets the reminder frequency and the tools that keep patients engaged, including automated push notifications and AI agent text messaging.
Who onboards patients, and when does it happen in the visit?
Onboarding always happens at each site, even when monitoring is central. The patient is in the building, and clinic staff can introduce the program. Someone at the front desk, a medical assistant, a nurse, or a provider may introduce RTM. The timing needs to be decided. It could happen in the waiting room at the start of the visit, when the patient is brought back with the provider in the room, after the provider visit, or at checkout.
Actuvi works out each clinic's workflows internally, then trains staff for their roles.
What roles and system access does a multi-site group need?
A multi-site group has more roles than a single clinic. Actuvi defines each one before launch: program administrators; clinic and site leads; clinical staff such as doctors, nurses, physical and occupational therapists, case managers, and medical assistants; administrative staff; billing personnel; and, when the group has one, an IT liaison.
Roles and permissions matter most here. Each person sees only the patients and tools they need for their job. Setting system access correctly at the start keeps a growing group organized as more sites join.
How should the rollout be sequenced?
The group chooses whether to start with one clinic or several. Training follows that choice. Sites with matching workflows can train together in one session. Sites that engage patients differently have their own session because training should reflect how that clinic works.
Actuvi delivers training in virtual live sessions and schedules them around staff, including those who work across locations or varied shifts. Sessions can be split into shorter blocks or smaller groups. Training covers platform administration, patient enrollment, assigning care pathways and assessments, managing alerts, communicating with patients, and extracting billing data.
What incentives keep every site moving?
Although optional, many groups add a staff incentive program. Incentives can be tied to successful patient onboarding, sustained engagement, and the education and setup codes that come with starting a patient on RTM.
Incentives can fast-track rollout as sites compete for rewards.
What happens after launch?
Support does not stop at go-live.
We assign each client a customer success manager who reviews the program monthly, identifies areas for improvement, flags potential issues, and tracks program performance to ensure the success of the RTM program for our clients.
Actuvi sends regular monthly or quarterly reports so the program keeps improving and each site gets the attention it needs.
Actuvi is built for multi-site clinics and hospital networks
A large organization runs many office locations, and Actuvi supports them all in one place, with controls that make a remote therapeutic monitoring program easier to run across sites.
Actuvi integrates with Epic, eClinicWorks (ECW), and other major EHRs, and meets ISO 27001 and ISO 13485 requirements. The platform supports FHIR standards, complies with HIPAA and GDPR, and uses role-based permissions to keep each staff member assigned to their patients. Automated tracking of monitoring time and data transmission days feeds CPT-coded billing reports, and automatic audit reports keep documentation ready for review across every clinic.
